I, the undersigned (Participant), acknowledge that I have received and understood this agreement and agree to comply with all the terms and conditions set forth herein.

Program Agreement:

As a condition of your court-ordered release or probation (the "Supervision Plan"), you are required to participate in the National Pretrial GPS Monitoring Program utilizing SCRAM Systems (the "Program"). By signing this agreement, you with this agree to the following terms and conditions:

1. Device Usage:

• Wear the approved SCRAM GPS device at all times.
• Charge and maintain the device in working order.
• Report any damage or malfunction to the monitoring agency immediately.
• Allow authorized personnel to inspect the device upon request.

2. Curfew and Territorial Restrictions:

• Live at the above provided address unless a change is authorized.
• Adhere to curfew hours as specified above.
• Do not enter prohibited areas as per court order.

3. Compliance:

• Follow all instructions from your probation officer or pretrial services agent.
• Notify the agency and the court of any changes in address, phone number, or other contact information.
• Turn yourself into the appropriate detention facility if ordered.

4. Financial Responsibility:

• Pay the specified daily monitoring cost.
• Return all equipment in good condition; you will be charged for damages.

5. Legal Consequences:

Any violation of this agreement or the Supervision Plan may result in legal consequences, including the possibility of bond revocation or contempt punishment.

Participant Authorization:

I, the undersigned participant, with this, acknowledge and agree to comply with the following rules and regulations of the monitoring requirements:

1. Device Possession and Maintenance:

• Wear an approved device at all times to verify compliance with detention or pre-trial release conditions.
  Maintain possession of the device on or near your person at all times.
• Verify identity and location as required by the court order or monitoring agency.
• Charge and maintain the device in working order and report any damage or malfunction within two hours.

2. Reporting and Compliance:

• Abide by other terms and conditions set by the monitoring agency.
• Turn yourself into custody upon the order of the monitoring agency or law enforcement.
• Notify the agency and the court of any changes in address, phone number, or other contact information.
  Comply with all additional conditions of the court order, including location restrictions.

3. Prohibited Actions:

• Removing, tampering with, damaging, destroying, shielding the signal from, or otherwise circumventing the monitoring device is unlawful.

4. Consequences of Non-Compliance:

• Non-compliance with any provision of this authorization or additional court order conditions, including location restrictions, may result in bond revocation or contempt of court.

Prohibited Actions and Penalties:

It is unlawful to remove knowingly, tamper with, damage, destroy, shield the signal from, or otherwise circumvent the monitoring device. Violations may result in legal penalties under Section 24-13-425.

Health and Safety Notice: Improper installation or use of the SCRAM GPS device may cause injury. Consult a doctor if you have any medical conditions that might be affected by wearing the device. Contact your supervising authority immediately in case of a medical emergency or device malfunction.
